Liability Waiver & Assumption of Risk

Close Quarters Boxing Club

By participating, or allowing my child to participate, in boxing classes and/or fitness activities at Close Quarters Boxing Club, I acknowledge and agree to the following:

1. Voluntary Participation & Acknowledgement of Risk

I understand that boxing and fitness activities involve physical exertion and carry inherent risks, including but not limited to injury, serious harm, permanent disability, or death. These risks may arise from my/my child’s actions, the actions of others, equipment use, or the condition of the premises, including risks that may not be known or reasonably foreseeable.I voluntarily accept and assume all such risks associated with participation.

2. Health & Medical Responsibility

I confirm that I, or my child, am physically fit and have no medical condition that would prevent safe participation. I understand it is my responsibility to inform Close Quarters Boxing Club of any relevant medical conditions. I acknowledge that Close Quarters Boxing Club reserves the right to request a medical clearance or management plan from a registered medical practitioner if there are any known or suspected medical conditions or concerns that may affect safe participation. Failure to provide the requested documentation may result in participation being delayed or refused until appropriate clearance has been received.

3. Release of Liability

To the fullest extent permitted by law, I release and discharge Close Quarters Boxing Club, its owners, coaches, employees, trainers, and volunteers from any and all liability for injury, loss, or damage arising out of or connected with participation in any classes or activities, including the use or misuse of equipment.

4. Emergency Medical Treatment

In the event of injury or medical emergency, I authorise Close Quarters Boxing Club to seek and arrange medical treatment on my/my child’s behalf if necessary. I acknowledge that all costs associated with such treatment, including transport, are my responsibility.

5. Personal Property

I understand that I am responsible for my/my child’s personal belongings and agree that Close Quarters Boxing Club is not liable for any loss, theft, or damage to personal property.

6. Rules & Conduct

I agree that I, or my child, will follow all rules, policies, and instructions provided by Close Quarters Boxing Club and its staff at all times.

7. Waiver of Claims

I acknowledge that I, on behalf of myself/my child, my heirs, and representatives, waive any and all rights to claims, demands, or legal actions against Close Quarters Boxing Club in relation to participation in its activities, whether such claims arise now or in the future.

8. Acknowledgement

I confirm that I have read and fully understand this Liability Waiver and voluntarily agree to its terms.
